Application for member

If you want to become a member of Urogynaecological society, follow these instructions:
 

1. Download an application
   Click on link below with right mouse button. In menu, which will appear, choose „Save as“ and choose place, where application should be save in your computer.

2. Fill in an application
   Open a file „prihlaska-ugs.doc“ in MS Word, print and fulfill.
    
3. Send an application
   Filled-in and signed application send on address of association (see Contact) Contact).
